By completing this form and making payment, you confirm that you have read, understood, and agree to the following terms. In consideration of my participation in the Tap Classes with Kimberly Chan, I confirm that I am in good health and physically fit to participate. I understand the nature of the activity and acknowledge that my participation is voluntary. I accept full responsibility for assessing my own fitness and agree to stop participating immediately if I believe conditions to be unsafe. I understand and freely accept the risks associated with dance activities, including risks not currently known or reasonably foreseeable. I assume full responsibility for any injury, disability, death, loss, cost, or damage arising from my participation. I agree to indemnify and hold harmless Kimberly Chan, her staff, assistants, and representatives from any claims or liability arising from injury (physical, emotional, or otherwise), loss of or damage to personal property, or any actions deemed neglectful while participating in the course. Kimberly Chan accepts no liability for loss or damage to personal property, whether caused by negligence or otherwise. A zero-tolerance policy applies to abuse, aggression, or inappropriate behaviour towards Kimberly Chan, or other participants. Breach of this policy may result in removal from the class without refund. It is my responsibility to inform the teacher of any existing injuries, medical conditions, or physical limitations prior to each class. Any injury sustained during a class must be reported immediately. I acknowledge that classes may be photographed, filmed, or live streamed for promotional and operational purposes across Kimberly Chan’s platforms. By participating, I consent to such use unless stated otherwise in writing before the course begins. All fees are non-refundable once payment has been made. Refunds will only be issued if the class is cancelled by Kimberly Chan. No refunds, credits, or pro-rata adjustments will be given for missed classes or inability to attend. Classes may be cancelled or postponed due to insufficient enrollment or unforeseen circumstances.
